Missing records to document weight
Good Morning All,
I have requested my medical records in writing several times to document my weight in 2004 from a former pcp. The ladies in her office are especially nasty and I believe they are dragging their feet because....well, they can. Is there another alternative to documenting my weight? I can't remember the name of the gyno I saw that year (yes, I've been dr. hopping I've just found a team of dr's I like in this city, and that's important to me). I have my med records from 2000-2006, only 2004 is missing. Could I perhaps submit photos of myself from that year as a testimony that I didn't miraculously lose and gain 100#'s that year?
Call the office and ask what you have to do to get your records and if there are any fees for it. If you don't want to do it you can have a friend call. Also find out if there is a time limit in which they are required to respond to your request. Are they affiliated with any hospital in the area? If so call the hospital and tell them what is going on and they may look into it and do something. Tell them that you want a copy of your complete file so they don't try to send you just a partial file. I would send the letter via Certified Mail with Return Receipt Requested. You can also have Restricted Delivery so that only the doctor can sign for it. It may be that he doesn't know what is going on. If they don't send them then, send a second letter and tell them you will be contacting an attorney, the state medical board and any other state/regulatory athorities you can think of if they don't send them. It might seem a bit drastic but they don't seem to care.
You could try having a friend make an appt with the doctor and you can go with her and tell the doctor what is going on. Of course that assumes the doctor doesn't know what is going on. If she does that won't work.
Or just show up and start telling everyone in the office what is going on. That will get their attention.
As for the gyno, can you call your insurance company and get them to tell you the name of the doctor?
