1. Which insurance company do/did you have?
2. How long were/will you at your place of employment when you took your leave for the surgery?
3. Did/Does your insurance require a 6 month supervised diet?
4. Did/Will your secondary insurance pay your co-pays?
5. Did your company have to grant you your leave, or did/does it depend on the length of your employment?

2. I've worked at the same place for 4 years
3. I didn't have to have any prior approval from my insurance. It's a covered benefit.
4. I don't have secondary insurance.
5. My leave was covered as part of FMLA. It's a federal law that they cannot fire me while I was on leave.

3. No.. but BCBS (at least here in NC) is re-vamping their requirements for WLS in August... you may want to look into to
4. I dont have secondary... but it depending on the coordination of benefits. If they pay what your insurance would pay, they will not cover additional
5. I am using vacation time... since I havent been an *employee* a year I do not qualify for FMLA. I do have disability but they have a 12 mo. pre-existing clause
If you are going for an interview and get hired I would tell them upfront about needing the time off later on down the road

3. Did/Does your insurance require a 6 month supervised diet? They wanted 5 years of Dr's visits with my weight on them, which in turned showed my ever failing diets.
4. Did/Will your secondary insurance pay your co-pays? i had a hospital co pay of 300, and 600 for surgeon which i got bacvk after insurance paid the dr.

Did your company have to grant you your leave, or did/does it depend on the length of your employment? no they didnt have too, but what was great was they new i was trying for it, so they sent my paperwork in (drs office) i was approved that day and had surgery 5 days later and i called work as soon as u found out and i was able to get off for 2 weeks and if they felt i could handle comming back then, then they would let me come back... which i did go back after 2 weeks

3. It required 6 months of prescribed treatment- I had previous records so I used those.
4. No.
5. Yes, but since it was a surgical procedure I wasn't able to return to work without a release from my doc.
