I too am currently experiencing weight gain - LW 148; Maintained Weight 150-155 for 4 yrs; Current Weight 192-198. Im scared to death to enter 200's. I admit I haven't disciplined myself (junk food junkie - esp. chips and sweets). I will begin walking today and try to eat more protein and veggies.

I appreciate your suggestions. I just recently had labs and all is well. I hadn't thought about mechanical failure, but certainly worth looking in to.

Not really unless you value your health :)

Seriously, you need to take your vitamins or the consequences can be bad.  I learned the hard way.

Did your surgeon give you a list of vitamins to take?

The American Society for Metabolic and Bariatric Surgery says all RNY folks need two multivitamins a day, 1500-2000 mg calcium citrate, iron, and B12.  They also say RNY patients usually need extra D3, at least 3000 iu a day and they say a B complex is optional. This is forever.  Your surgeon should have discussed this with you.

These are the minimums.  You should also have regular labs done and supplement anything that is low.  Keep records and watch for anything trending downwards.  Don't wait until you are deficient as it will be much harder to catch up by them.

It really shouldn't be that expensive.  I stock up during sales, buy one, get one free deals.  Check online for good prices.

And it is also important that you get in enough protein.  Believe me, you will regret it if you don't.

Many can eat enough to meet their needs.  I still need to supplement to get in enough.

There are some places that will give you samples but not enough for a long time.  There is no need to buy expensive bariatric vitamins but make sure that fat soluble vitamins like A, D, E and K are in dry form since we don't absorb fat as well after surgery.  And make sure your B12 is sublingual, a shot or spray since our stomachs no longer have the intrinsic factor needed to absorb it.

I would guess I spend very little a month on vitamins because I have a stock pile from sales.  Whenever the store near me has a buy one, get one sale I will buy more to have on hand.  Protein supplements can get spendy but I make due with Body Fortress from Walmart at $15 a canister.  I also eat a lot of chicken, which is cheap and loaded with protein.  Walmart often has their cooked chicken marked 50% off, so for about $2 I get an entire cooked chicken.  I have a food sealer so I vacuum pack it and it lasts forever.  That also help me buy in bulk when there is a good sale on meat.

Don't stress it.  It really isn't as daunting as it sounds at first but it is essential.

My first thought is B12 deficiency over Vitamin K.

Vitamin K deficiency is rare. It is more likely that you would have a B12 problem due to the RNY and lack of intrinsic factor (I am assuming you know what that means in relation to needing iron infusions)

A PT test should be 9.5 - 13.5 seconds. Normally aPTT tests in a person with prolonged coagulation (such as someone on Heparin) is 2.5 times that in  seconds. So it looks like your doctor is focusing on that 2nd test.

Did the doctor def state Vitamin K?

There are several disorders that can also lead to a high aPTT. Liver disorders are pretty common causes. Have you had liver studies done?

These are common liver function tests:

  • ALT. 7 to 55 units per liter (U/L)
  • AST. 8 to 48 U/L
  • ALP. 45 to 115 U/L
  • Albumin. 3.5 to 5.0 grams per deciliter (g/dL)
  • Total protein. 6.3 to 7.9 g/dL
  • Bilirubin. 0.1 to 1.0 mg/dL
  • GGT. 9 to 48 U/L
  • LD. 122 to 222 U/L
  • PT. 9.5 to 13.8 seconds

Hi there!

I am 3 days into "starting over" !!  My surgery was 2/5/08 and I had lost 130 lbs. I have gained back about 30lbs and I don't like it either! So what to do what to do!?

Here's the short version of what I am doing (and I'll be very detailed in a bit)

1. MyFitnesPal app on my phone for a reasonable outline of food intake

2. Syntrax Nectar protein powder

3. Mild exercise (I am 9 weeks post-op from a major foot surgery - so when I say mild, I mean MILD)

It didn't seem reasonable to go back to the immediate post-op diet as I know I couldn't handle doing that just yet. Not saying that anyone here is wrong for giving you that type of advice. I have GIVEN that advice myself to "other" people but the thought of how restrictive that is, has put me into a procrastination mode for the couple years. It's not that I am gaining... I am managing that 30 lb increase just fine.

I may actually go back to a modified version of the post-op diet if I can stay on this for at least 30 days. By that time, my mindset will be different.

My Fitness Pal: (Android and Ios)

What it does: Daily allowance of calories based on weight, amount wanted to lose, and accurate nutritional intake (broken down quite extensively)

This smart phone app was a real eye opener! I had no idea how much I was consuming and how little water, protein and vitamins I was getting. One example is I like decaff coffee in the morning, no sugar, half-and-half. My half-and-half consumption for the day was 160 calories!!!!!!   JUST the half-and-half!

So I am now very conscious of what I am eating every meal because I know what I just ate and how many calories I have left.

Protein powder:

What it does: Increases protein intake, decreasing overall appetite, heals muscle, bone, etc. V. important in WLS (as we all know)

The thought of chalky Vanilla and Chocolate protein shakes made my stomach churn so I found that Snytrax Nectar whey protein isolate mixed with zero calorie flavored water was the perfect option. I bummed a bit when I realized it had 100 calories a serving but after three days of having protein powder for lunch, I found I wasn't hungry until dinner time.

Mild exercise:

What it does: Gives me more calories to eat (yum) and increases endurance, strength and improves my mood. Also, skeletal exercise is needed to protect bones from Ca loss, which is a biggie for me, being 47 and menopausal.

Like I said: MILD. I am barely walking without the CAM boot yet from foot surgery on June 20th. Before that I was in pain since October and barely walked either. SO many people would say "well, you can lift soup cans" and I wanted to smack them in the face. It was DEPRESSING to not be able to walk and even go on an errand to the grocery store.

So I am able to walk my dog 2 blocks right now. (Note: not AROUND the block -- a straight line of 2 blocks) It takes me 15 minutes. I also have to do knee bends, balancing, isometrics and strengthening exercises to allow me to walk better. That takes about 15 minutes. So that is 30 minutes a day of MILD exercise.
