  Try to go to an urgent care or someplace else and get an EDG if you are even slightly dehydrated or having bad pain like that you may need some thing like carafate. That might coat your pouch and might avoid surgery. I am no doctor, but my experience was that any ER can do an Ultasound and any internist can order an endoscopy to be done at an endoscopy center, depending on your insurance. You do NOT want to let this go. SMOKING will KILL you because it will cause your stomach and your pouch to basically turn to tissue paper and then when you eat or drink any thing  it's like trying to hold a protein shake in a kleenex. That's why the doctor isn't paying attention to you. It's a liability issue. If you want to find out what's going on, try a different medical group and get scoped STOP SMOKING and take the medicine. or you already wrote down what will happen. There ARE ways to heal ulcers and perforations but it is VERY hard. Good luck.

Been there, done that myself.....TWICE!  You'd think I'd learned the first regain period but nope.  It took a second regain of 31 pounds to get my attention, take a hard look at myself and change some things.

As you are finding out, losing regained weight is a slow and frustrating process.  But, it does come off.

I logged my food for a long time.  That helped me because if I didn't want to see it, then I didn't eat it.  Whenever cravings came up, I used my Distraction List until they passed.  (I created a Distraction List similar to a check list of things I could do when triggers and cravings came up.)  I also found recipes and food items that were different from what I used to eat, I enjoyed and healthy.

I just had a total knee replacement a few weeks ago so my activity is physical therapy.  However, I look forward to being able to take walks and walk on my treadmill.  

Emotional eating has been an issue so I stay very aware and not dive into ice cream or cookies to stuff my emotions away.

Lastly, have confidence in yourself that you've got this....because you do.

Several surgeons have stated that when they go looking to see if there's a mechanical problem with the surgery, they find none.  People have just stopped following the post-op plan.  It's unlikely you've stretched your pouch.  It's possible your stoma has enlarged.  But it's more likely that you're just not eating right!

Stop drinking with your meals.  If diet soda gives you the munchies, stop drinking it.  Dense protein (chicken breast) first, veggies second, some good fat.  Keep refined carbs to a minimum.  Track your calories (myfitnesspal) so you're accountable for what you're putting in your mouth.

Transfer addiction is real and you need to get a handle on the drinking, either by yourself or with the help of a therapist or the help of an organization like OA or AA.

I would start by calling the number on the back of my insurance card and finding out what my mental health benefits are.  Then I'd get a list from them of providers and start calling and making appointments until I found one I clicked with.  Fully expect that it might take some trial and error to find the right one, but the right one for you is out there.  

If you don't have insurance, but are employed, see if your HR offers any sort of employee assistance program that has health care options.  

And if those aren't available, try looking for low cost counseling in your area, or even clergy if you're a member of a church.

Just fyi, I didn't realize how off my signature is! Ignore it! My current weight bounces around between 200-206, but I made it all the way down to 192 for a day or so, LOL! I'm hoping to get back there, and then go down from there.

Cindy, I just want to tell you how much I admire you for surviving all you're going through.  A lot of people couldn't handle all the stress you're experiencing, yet you're hanging on. I'm a very strong person, but I don't think I'd last long, if I were in your shoes.

All that being said, and keeping in mind I'm sticking my nose where it doesn't belong, I think you need to do what everyone else has said, and find a therapist or counselor who can help you work through all the emotions and pain you're feeling that are resulting in the self-medication with food. I also think you need to realize that you ARE NOT responsible for anyone else's happiness, only your own. I'm from the generation where women were taught to cater to their "men-folk", but I've learned along the way that's a bunch of hooey! = : P  If your husband is ambulatory, he should be able to make his own meal, given the fact you're working 12 hours a day. Again, this isn't my business; just adding my two cents! More than anything, I hope you can take a couple of steps back, and realize YOU ARE WORTH TAKING CARE OF YOU, as well as you take care of others. 

For what it's worth, I'm almost 2-1/2 years out from my surgery, and after a 120 lb. weight loss, I've re-gain about 10 lbs., and even worse, I'm falling back into my old ways.  A lot of it is stress, and like you, I'm eating things that are bad for me, even though I know I shouldn't. Even worse is that NONE OF IT TASTES GOOD! In my mind, I want and need it, but the reality is afterwards, I often want to through up, because it just doesn't feel right in my body, if that makes any sense. And many times, I dump, but keep eating the wrong things anyway! How sick is that?

Your best bet would be to find a local surgeon (if your surgeon isn't still around) and get checked out.  Make an appointment with a nutritionist or dietician and get an eating plan.  You can get back on track and lose the regain.  Are you seeing a therapist to help with the alcohol?  Transfer addictions are very common post-op.  Sadly :(

You should definitely work on finding on you click with.  It could really help you deal with your feelings of depression and failure and help you to get back on track.
