They gave you rx vit D, though, right? And you are RNY, so you malabsorb fats/oils to a degree. AND the rx is D2, which has to be converted to D3. Our converters are bypassed, hence very little response to the rx D2.
The job of the PTH is to distribute incoming calcium. It gives first to the soft tissue to fulfill the primary objective: keep the heart in rhythm. Any left overs go to the hard tissue, bones, teeth, etc. As long as there is plenty of D and calcium coming IN, the PTH level will remain low-ish. If there is NOT enough, the PTH turn up the heat, the level goes up and they start shoveling calcium out of the "less necessary" places to fulfill the primary mission.
If there is a tumor, it throws all the numbers off. So, it needs to be looked at to see if it's causing trouble with your bones or is dangerous. But if there is one nodule on only the 4 PTH, unless it is dangerous, I hate to see ppl kill the messengers! The PTH are there to protect heart and frame, so when they go high, they are screaming their emergency message out to get more D/calcium on board. If they zap them all, without a good reason, they have killed the guards/messengers/turned off your security system, so you have one less area guarded against disaster.
Tumors are medical and need to be addressed as such, but some docs want to kill the PTH without MRI or CT, just based on high PTH levels. Before they zap 'em, please ask 100 questions and make sure that ALL FOUR need to be killed, ok?
