It wouldn't let me cut-n-paste it properly either...hope it will make sense. Lab Tests recommended for Gastric Bypass patients Regularly (every 3-6 months depending on your previous results) Keep track of any levels that are declining from one set of tests to the next, even if not yet in the low range. Make any corrections necessary in your diet and vitamin regimen before the levels get too low. CPT Code Test Description 85025 CBC w/diff Complete Blood Count with Differential Test includes: Hematocrit; hemoglobin; mean corpuscular volume (MCV); mean corpuscular hemoglobin (MCH); mean corpuscular hemoglobin concentration (MCHC); red cell distribution width (RDW); percentage and absolute differential counts; platelet count; red cell count; white blood cell count 80053 CMP Comprehensive Metabolic Panel Test includes: A:G ratio; albumin, serum; alkaline phosphatase, serum; ALT (SGPT); AST (SGOT); bilirubin, total; BUN; BUN:creatinine ratio; calcium, serum; carbon dioxide, total; chloride, serum; creatinine, serum; globulin, total; glucose, serum; potassium, serum; protein, total, serum; sodium, serum 80061 LP Lipid Panel With Total Cholesterol:HDL Ratio Test includes: Cholesterol, total; high-density lipoprotein (HDL) cholesterol; low-density lipoprotein (LDL) cholesterol (calculation); triglycerides; very low-density lipoprotein (VLDL) cholesterol (calculation); Total cholesterol:HDL ratio 82977 GGT Gamma Glutamyl Transpeptidase (GGT). Use: A biliary enzyme that is especially useful in the diagnosis of obstructive jaundice, intrahepatic cholestasis, and pancreatitis. GGT is more responsive to biliary obstruction than are aspartate aminotransferase (AST) (SGOT) and alanine aminotransferase (ALT) (SGPT). 83615 LD or LDH Lactic Acid Dehydrogenase. Use: diagnosing anemias, hypothyrodism, liver diseases. 84100 PO4 Inorganic Phosphate, Blood. Use: parathyroid issues, liver disease, malnourishment, vitamin deficiencies, osteomalacia, 84550 UA Uric Acid, Serum. Use: gout, dehydration, anemias, low protein intake 82607; 82746 B12 & Folate Vitamin B12 and Folates levels. 83540; 83550 Fe & TIBC Iron and Total Iron Binding Capacity (TIBC). Test includes: Percent of saturation; serum iron; total iron binding capacity; unsaturated iron binding capacity 82728 Ferritin Ferritin, Serum The serum ferritin is, other than a bone marrow examination, the most reliable indicator of total body iron stores. 84446; 84590 Vitamins A&E Vitamins A & E. Requires patient to fast for 48 hours prior to collection, Specimen must be protected from light and frozen. Use: check for vitamin deficiency 82306 Vitamin D Vitamin D, 25-Hydroxy (Cholecalciferol Metabolite; 25-Hydroxycalciferol; 25-OH-D; Vitamin D3 Metabolite). 84436; 84443; 84479; 84480 Thyroid Panel Thyroid Profile II (Comprehensive) Test includes: Free thyroxine index (FTI); T3 uptake (THBR); thyroid-stimulating hormone (TSH); thyroxine (T4); tri-iodothyronine (T3) 84630 Zinc Zinc, Serum. Use: monitor nutritional deficiency 83970 PTH Intact Parathyroid Hormone (PTH), Intact. Requires overnight fast. Use: Diagnosis of parathyroid disease and other diseases of calcium homeostasis. PTH is secreted in response to decrease in serum calcium levels by increasing the renal reabsorption of calcium and lowering reabsorption of phosphorus. 83735 Mg, RBC Magnesium, RBC. Use: evaluate magnesium deficiency 83036 Hgb A1c Hemoglobin (Hgb) A1c With MBG Estimation. Used to estimate mean glucose levels over time. *YEARLY - DEXA SCAN for bone density. Diagnosis codes: 579.3 post-surgical malabsorption 268 vitamin D deficiency 269.2 hypovitaminosis 268.2 metabolic bone disease 244.9 hypothyroidism 250.0 diabetes 579.3 short bowel syndrome 401.9 hypertension
