Do you have first choice insurance w/exclusion on bariatric but were approved anyway?

Hi, I am an ideal canidate for bariatric surgery but my insurance has an exclusion on bariatric surgery. I'm seeing in a few instances under the insurers where there was an exclusion but they paid anyway. My insurance is first choice, They are administrators for Good Samaritan where I work. They have an exclusion for bariatric surgery, something I don't understand. With my health problems they are paying alot every year for meds and Dr visits all due to my weight issues.
