Ins. approval w/written exclusion???

Has ANYONE been approved for surgery when the insurer has a written exclusion? My insurer "updated" the policy while my request was enroute. Which means that I did not find out the surgery was no longer covered until a month after receiving my first denial. Ouch! What next? Time to hassle the employer, send flowers to the ombudsman?
