Psych eval doc needed

My sister is scheduled to have her surgery in 2 weeks and her Blue Cross of NY has just now come up with the hoops she has to jump through to qualify for insurance coverage. They want her to have a Psych eval done before her surgery. Does anyone know a doctor in the Puget Sound area that is obesity friendly that can do the evaluation? Your help is greatly appreciated.
