Wish Center Question

Hi I forgot when you want to go on the online Wish Center site the patient only site they gave us a pass word and a user name, I thought it was our last name and our social # for the pass word does any one remember. also Do we have a 6 month check up I have to dig out all my paper work and can't remember.I remember the 2 weeks , 3 months but I can't remember if it goes to a year then for a check up. No 6-month check. We have 2-week, 3-month, then 1-year, but don't forget all the support group meetings each month. I don't know about the password; I never got it even though I am beyond 3 months out!! If someone knows, please post. Thanks, Chris
