First Choice Health Network anyone?

I'm a new member of First Choice Health Network (PPO), is anyone familiar with them? Does anyone or has anyone had the surgery that First Choice has covered?

I have First Choice Health Network PPO. I was approved for RNY last Friday. In my case they used Innovative Care Management for pre-authorization. I had to prove medical necessity by providing some lab results (diabetes), my latest sleep study (sleep apnea), results of a psych evaluation (required) and letters from my PCP and surgeon. I have a number of co-morbidities and had successfully lost weight and then regained it three times in the past. I had absolutely no problem being approved - my main problem was getting the surgeon's office to submit the proper paperwork in a timely manner.
