PEBB folks w/Pacificare

I would greatly appreciate any information anyone can give me relating to any approvals for PEBB members who have, or have had in the past few years, Pacificare insurance. They've started enforcing the PEBB written exclusion policy that's existed for years; they used to ignore it. I need to be able to show that it's their policy that's changed, not my insurance, for the appeal process. Thanks for the help.
