Group Health - Options

I've been doing research on surgery and know that I want to pursue this with my PCP. I don't see any info about Washington State folks who have GH-Options insurance. My book says that it's covered, but I also saw a limitation on amount available in a 2 year period which is less than $12,000. I'd love to hear info from anyone who has experience getting approval from Group Health.
