need phone number for eval!!

I am having a revision done and need to have another psych eval done- not a full one just something that lets them know I'm sane I guess. Anyway, if anyone has the phone number for Christine Rasmussen that would be wonderful . I had my first eval done in Lakewood 3 years ago but they don't do them anymore and the gal I saw is now gone.Any help you guys have would be great. Thanks so much Sharon
