if your employer "opts out" of buying coverage for weight loss surgery for its employees ,no letter of appeal or anything else will make the insurance company pay for it. Not even medical necessity.

The insurance company doesn't have to pay for a surgery that your employer hasn't bought the coverage for.

i suggest you look into self paying in the states or in Mexico, where many in this situation have gone, myself included.

I work in insurance pre-auth's and I can tell you that if your employer opted out of bariatric surgery coverage on your insurance plan, there is no amount of appealing it that will get it covered.
I am sorry to have to break the news to you, but they will just tell you it is not a covered benefit on your plan.

The best advice I could give you would be to either look into self-pay options, financing, or surgery in Mexico which can be significantly less than what you would pay in the US.
check the bariatric surgery in mexico forum and get some referrals or ask questions you may have there. LOTS of people do it, and it can even sometimes be more affordable for people who do have insurance that covers WLS because of high deductibles.

good luck and I hope that you find a way to get the surgery you desire!

Appealing to your employer's HR is the only appeal that might be useful in this situation.
