I don't know where you are but the hospitals here mostly have finance departments where you can spread payments over 2-3 years.
Lots of folks go to Mexico. Pick a place with a good reputation, and you'll get excellent care, and a great price to boot. If I were doing self pay, that's what I would do.

My credit score wasn't the best when I applied for my loan. I think it was like 650ish though? There are a few options.
- If she has a nicer car that is paid off they will let her take a loan out using the car as collateral. I didn't want to go through route because I'm paranoid (what if something happens & I can't make payments? I can't lose my car!).
- I went through a lender that didn't require collateral, LendingUSA. They have an A+ rating with the BBB so that's why I felt good about using them. They have been a little difficult to work with (my lending agent was IMPOSSIBLE to get a hold of and I signed up for auto draft payments and somehow it didn't work). But apparently everyone else that uses them loves them so maybe I'm the exception & I wouldn't want to dissuade anyone from using them just because of my experience. My APR is very high (like 22%) but even with that I will still be paying less than I would in the states because I'm going to Mexico. That's how I justified it anyway. haha
- There are also several places in Mexico that will finance. I think Pompeii Surgical is a popular one.
There are soooo many options out there. If she wants it badly enough she will find one that works for her!
