Everybody's insurance is completely different. I have BCBS AZ PPO (I'm in Tucson) but the rules for my policy could be completely irrelevent to what you're going to go through. Your best bet is to find a copy of your benefit book and read through the coverage requirements for surgery, or call the customer service number on the back of your ID card to ask.
That said, I attended my first information seminar in late August, did a 3-month supervised diet while I rounded up 5 years' medical records and had some tests done, and had surgery in December.

SparkleKitty is right...I have BCBS in Missouri, but it all depends on your employer. Each employer can put restrictions and requirements on the plan that another doesn't, so the timeline is different for everyone. Mine required a 3 month doctor supervised diet prior to surgery in order to get approved, others in Missouri did not. Just do all you're required to do and be patient! Best of luck!
