Approval w/out diet? ?

My insurance required documented attempts at previous weight loss but no weight loss requirements before surgery. My surgeon required that I lose 30 pounds before surgery. The best I could do was maintain the same weight. The "requirement" was more like a "guideline." He still did the surgery.
