Why not be upfront? Your boss etal are going to figure it out anyway, relatively fast, when you lose so much weight so quickly, or, they will think you have a terminal illness. Either way the questions will begin.

Just say you need time off for a surgical procedure. They aren't supposed to ask and you are not obligated to give them anymore info than that.
