Thanks. I decided today that this is the route I'm going to take. I'll be honest and up front and you're right I can postpone until a better time but somehow I think they'll be okay with it.

I made a lateral move right around surgery time and also already had a vacation planned  and I just told the truth which was,  " I'm having surgery." Didn't say what kind and didn't need to (they didn't ask). Didn't need hr. We have PTO so it's all time off but I doubt you will have issues with work. I took 2 weeks but could have taken a week fine.

As a manager, I think you should just be up front and let them know that you have a medical procedure planned.  Don't forget to get your FMLA paperwork filled out.  This will protect you.
