My daughter has chosen Plication. Could use some feedback on this.

I thought of the pilcaton as well but researched it ( I changed my mind ) when I read about a person who had it and ended up having a polypup(sp) and lots of pain but they could not diagnose it as it was folded into the stomach... I used dr Oritz as well the clinic was nice went smoothly and no problems good luck

Did you try the Dr. Ortiz' forum at the OCC? There's a plication forum under VSG.

I looked into their forum a few times and get the impression it's very closely monitored for what people can post.
I'm a bit suspicious of that.

I know Dr. Ortiz can do the sleeve but I don't think he is very impartial about explaining honestly about it vs. plication.

I don't want my daughter steered the wrong way because he needs more patients for his study. Just my gut feeling.

Yeah, that's why I stopped using their forums... I think they delete anyone having problems...and there was some nasty peeps on the band forum when I used to go there, too. Dr. So was the one that explained the difference and at first he said kept mentioning the plication when I was trying to decide between the two, but when I finally said sleeve, he said that for band revisions, that's what they were recommending. I must say...when I went in for revision surgery, I had it on a Thursday (a dat they usually don't do VSG) and there were 3 people in there for "fixes" to their plications. One guy had popped the top fold open so it was unfolding like a peeled banana and one very young girl was having a revision from band to plication...there were 7-8 plication peeps the day I had surgery and all I could hear all night was dry heaving up and down the hall because everyone was sick and naseous...I don't know why I remember that. I had a little nausea but not like that! I also remember the horrified looks at my drain...

The main questiion that I have had since I first heard of the plication is why? What is it's advantage over the sleeve or other procedures that would make it attractive to establi****self in the marketplace? The sleeve has distinct advantages over the well established "gold standard" RNY, as does the DS, but other than theoretical reversibility, the plication doesn't seem to offer any particular advantage over what's already available to establi****self as a long term player in the bariatric market. It looks like this is a place where "early adopters" will wind up with an orphan procedure, with few doctors in the world who will understand its quirks and side effects long term - the RNY, despite all of its problems and limitations, has a large knowledge base from which doctors can draw in treating patients in the long term, as does the VSG from its history as a DS component and growing history as a stand alone procedure.

I assume people are attracted to the idea that leaks are off the table, if the stomach isn't being cut.  And supposedly reversibility.

But the stomach tissue, once folded onto itself, can grow together via scar tissue.  (There goes the reversibility.) Also, it could develop issues such as polyps, ulcers, possibly even cancer, inside the folded areas that are nearly impossible to diagnose.  And then there's the ever-present ghrelin.

All in all, a bad trade-off, IMO.

I will pass on what my doctor, one of the most respected bariatric surgeons in the States, told me about plication.  He said, that he would never do it and that it will not be done in the United States and only makes sense in the poorer countries.  The reason is that it is much cheaper to do since it does not use the titanium staples.  The surgery is irreversible, just like the sleeve, and it does not reduce ghrelin, and if there is ever a problem, it is almost impossible to have any revision done.  He is totally against it.  I would have serious concerns about your daughters decision.  I wonder if she could have a phone interview with Dr. Cirangle so that he can explain to her why this is such a bad idea.  I am sorry, I wish I had something more positive to say here.

I haven't researched this well at all, so take what I say with a grain of salt, but it almost seems a bit sadistic....

Think about having a stomach volume the size that we do right now, but still with our pre-op hunger.  That sounds horrible. :(  I'm not sure if that's how the plication works out, but if so, it sounds horrible.  I'd be miserable if I was constantly hungry.... oh yeah, I've been constantly hungry on diets in and out the past 20 years, and it was horrible and miserable.  

Good luck to your daughter whatever her choice.

From what I understand.....plication does nothing to remove ghrelin.  This one was one of the major selling points of the sleeve for me.  Just my two cents.
