After faxing the missing information, I was approved in 3 days. I would imagine it has to do with the reasons for the denial.  I was missing a letter from my PCP.

I like calling North Carolina home!!!  Go Tarheels!

I work in the medical field and do a lot of insurance appeals (not for surgery but for medical equipment). Anyway they usually have 30 days BUT you need to be proactive to make sure the MD calls and maybe even have him write a letter of medical necessity. It usually helps if you call as well and submit a letter that you want to appeal and why (why you think the surgery is medically necessary, maybe even include some research articles about VSG). I have had times where I appealed 3 times and then was approved, so dont give up hope right away. Keep on top of the doctor and your insurance company and make sure that you know what documents they want and do whatever you can to help. In my experience the MDs can take their time with appeals unless they are pushed. Good luck
