ON the CPT code...As of Jan 1, 2010...VSG is coded to 43775  It will ALWAYS be 43775 !
Its OFFICIAL..per AMA!

 I'm from LA - I also was a self pay - I used Dr. Hargroder in Gronzales - he was wonderful and so was his staff.  It was around $15,000.00.  Good luck.
