How do I change.....

How do I change my Dr. and my "waiting to have surgery" to a date? I've tried everything! Can anyone help?

Go to My OH at the top of the screen then chose My Account and follow the choices. It is under the middle category at the bottom of the screen...there is I have a date and add my surgeon both.
