I do! Unfortunately, though, my employer did not pick up the bariatric rider or whatever so they didn't cover my surgery at all... they do, however, pay for my nut pre and post-op, and my pre-op "life coach" as well.
Sorry - wish I could help! I really haven't dealt with them at all. I've had them for a while though and never had any problems. Never tried to get surgery approved either, though
