LETTER!!!

HELLO IT'S ME AGAIN.....I WAS WONDERING IF I COULD GET SOME HELP. I HAVE TO WRITE A LETTER FOR MY INSURANCE ADVISING WHY I WANT THE SURGERY AND WHAT I WOULD BENEFIT FROM GETTING THE SURGERY. DON'T GET ME WRONG I KNOW WHY I WANT IT, BUT I NEED SOME HELP PUTTING IT ON PAPER AND EXPRESSING IT LIKE I NEED TO. SO IF ANYONE HAS ANY EXAMPLES PLEASE EMAIL ME AND LET ME KNOW. THANKS A BUNCH,
