BCBS of TN (Bridgestone)

Could someone who's had the surgery w/the insurance from Bridgestone please tell me if they know if the DS is covered? I am going to make an appt for my consultation, but before I go & have to pay the 200 out of pocket I would kind of like to know. Also, if someone has had this surgery by Dr. Husted's office w/this type of insurance I would love to know how much you had to pay out of pocket & did you have to pay all the expenses up front before the surgery? Thank you!
