UHC and wls

I have a quick question. I have UHC community plan aka tenncare. What are their requirements for surgery? 

I also have this uhc tenncare 

I have completed a 6 mth physician supervised diet

Psych eval, nutritional eval and 2 Dr visit notes from each year for the past 5 years

Also a letter from my pcp requesting weight loss surgery for me. My bmi is 37 and I have 3 comorbidities so I should get approved for the sleeve. At least that's what I'm hoping for!
