I'm posting this for a student, Monica O’Rourke, a Ph.D. student at Middle Tennessee State
University. Neither I nor Centennial are involved in this study in any way, we will not have access to the results or have any knowledge of who does or does not complete the study.

She invites you to take part in her dissertation research study for my Ph.D. degree. http://www.surveymonkey.com/s/cto_activity_survey

This is an online survey on SurveyMonkey.com regarding your current attitude about physical activity and your middle and high school physical education experiences.  This study is an important one that will help health educators better understand what adults’
secondary school physical education experiences were like, their current
attitudes about physical activity, and whether physical education met their
needs.

SurveyMonkey is a trusted survey site that many companies and universities
use to conduct research.  Your answers and e-mail are *completely
confidential and responses are anonymous and cannot be traced to any
specific person. *Therefore, I encourage you to be very candid in your
responses.  It will take approximately 10 minutes to complete.  Would you
please participate?

Here's the link to the survey.
http://www.surveymonkey.com/s/cto_activity_survey If you have the time,
please complete the survey now.  Yet, I know it is the holiday season so the
last day to complete the survey is January 10.

Thank you for your time and consideration. It is only with the generous help
of people like you that my research can be successful.  I encourage you
share the survey link with friends and family that may want to participate.
If you have any questions, feel free to call or e-mail me.  Thank you in
advance.

Respectfully,

Monica O'Rourke, Ph.D.
student
