You just get your doctor to write a note that you are under his care. Give a start date and a return to work date. That's all they need to know. If they can identify the doctor as a weight loss surgeon, talk to him about how you can still get a doctor's note from another doctor... Perhaps your primary care physician.

Don't litter! Spay or neuter your pet
Your doctor can just put down that you are having abdominal surgery. That will take care of any lifting restriction you may need for work.

P E A C E - It does not mean to be in a place where there is no noise, trouble, or hard work. It means to be in the midst of those things and still be calm in your heart.
If your employer gives you paid time off/ vacation time that you are planning to use for the time off, they legally can't ask you why you will be off or what you will be doing so you dont have to tell them anything. If you are using sick time or unpaid time then I would go with the other suggestions and just get a Dr's note.
