Cigna insurance
I have CignaPOS, TN state wmployee, and I was required 5 full years of weights. So when everything was submitted this past May I had weights back to 2004. Just make sure you have EVERYTHING before you even try and submit anything or they'll deny you, or at least they did me. Plus follow up with Cigna because they "didn't receive" my information even though we had fax confirmation, so after it's submitted make sure you call about a week later to check and be sure. Besides that it wasn't too difficult, especially if you have a hospital who works really hard for you.

I would recommend trying to find some pictures with some sort of date to show that even then you were overweight. I don't know if the letter will work or not, I had to dig and dig to get my weights back that far, one was a hospitalization, another was a pap smear. However, in your case a letter might work. I think it just depends on the reviewer and the mood he/she is in that day. But if you could find pictures that would definately help and wouldn't hurt.
Also go back and do your best to remember every diet you tried and failed just make a little chart showing date start date ending how much lost and what the out come was. Example:
Aug. 8, 2001 starting weight 265 "Slim Fast" Sept. 1, 2001 ending weight 252 lost total of 13 lbs. quit because couldn't stand to drink another shake regained all weight plus 4 extra pounds.
Try to put down each diet supervised or not. If you did a structured program like weigh****chers, LA weight loss, etc. then try and come up with the documentation this will strengthen your case and might suffice the weight loss history of 5 years. Good luck it can be a hard fight but don't give up and when you have ??? ask someone will help. C'ya Bob

I started the process in march but had been to the dr in December and January and February with my weight documented (for other health reasons) so that counted towards my 6 month diet...........
