Anyone have United Healthcare insurance??

After spending a year on requirements:
EGD
Gallbladder U/S
Psych eval
Support groups
Weigh ins

...I just found out that my company insurance is changing to United Healthcare.

So basically I do not know what the requirements will be until July and I'm just curious if anyone has any insight.

I'm actually having surgery June 30th and I have united healthcare insurance.  Psych Evaluation, 6 months supervised diet and that's about it.  also, if you smoke, you must stop  for 6 weeks.

It really depends on what policy your company gets. I have United Healthcare. My requirement was BMI over 40 for past 5 years. That it. I had it easy.

My UHC plan said I had to show 5 years med recs (one record for each year) to show I have been obese for 5 years. and a psych eval. I had an answer within a week.

I had United Health Care.   I had to have 5 years documented attempts to lose weight with BMI showing me 0bease.  If BMI was not high enough, there had to some other problems such as diabetes or sleep apnea. 

In addition, you might want to check if your employers has an exclusion in their UHC policy for weight loss surgery.  My UHC agent told me that sometimes there is an exclusion in the policies of some companies.  Luck was on my side and there was no exclusion in my policy. 

Your UHC agent should be able to tell you or the benefits office of your employer.

Once my Dr. submitted the request for approval, I got my answer in one week.
