Don't worry...it's a breeze! After you finish, you'll wonder what you were worried about. Beth is telling you exactly as it is...a standard bubble sheet. You'll be asked a bunch of questions (some relavant and some not so much) and the same 10-15 questions are peppered throughout the assessment.

I posted the same question and then had my psych eval. It was easy. Beforehand though, I mailed a packet of several pages of questions to answer and circles to fill in. Probably was like 375 questions. He reviewed the test results at my eval and asked questions about the surgery and what I knew and the possible complications and my committment to after surgery life and what kind of support I have family/friends. I'd say if you know the ins and outs of surgery then it's easy. Good luck to you :-)
You have probably already left for your appt... I will give you things NOT to do: don't twiddle your thumbs and rock back and forth or suck your thumb don't do a far away stare and after everything they ask, say "huh?"
don't stand up on the couch and scream out loud that they are coming to get you Ok thats pretty much all that failed me when I went! So as long as you don't do those things, you should be ok. lol
