5 year diet journal ???

I've heard of medical documentation of 5 years of obesity but never a 5 year diet journal. That's insane! I just happened to have a solid 2 year record of Weigh****cher's attendance and that blew my insurance company and my surgeon away. I was even shocked that I still had it. I think you need a new doctor. This one doesn't seem to be all that interested in really helping. I have to second the recommendation for Baptist Metabolic Surgery Center and Dr. Spaw! Where ever you wind up, I wish you a speedy approval and recovery. I hope you can find the surgeon and program that is best for you. Of course, we are always here for support too!!

I do think 5 years is a little extreme, I dont know that I had it documented that long.  Dr Houston at Centennial did mine and I recommend him highly.  All of the docs mentioned here are great doctors , just look at the results of their patients.  I would go to a new doc since you have new ins. anyway. LET me say I TALK/write a lot even though I can't spell!!!  Well I think I have GREAT NEWS!!! I called to set up an appointmet with the new PCP (Primary Care Practioner) I can't spell to save my life... anyway okay I called them but they aren't accepting anyone new so I spent the next 30min's (this morning) and called the insurance company got a list of some other doctors, called them they aren't taking new members, called the insurance company got more names, called the doctors they weren't accepting, called the insurance company got a bigger list of other doctors and then finally found a doctor at CENTENNIAL MEDICAL CENTER named Melissa Hixion set up an appointment to see her for Monday 1:30p.m. found out that she has and does do referals to Dr. Dyer and his associates.  I ALSO when I first called the insurance company they gave me a list of everything that they need me to do that are their qulifications.  I have done everything except for the 5 year info so I asked her specifically and she transfered me to another woman that was her supervisor and she transfered me to a woman that actually looks over and approves or denies people and she said basically this: What they want is a 5 years of medically written down paper's showing when I went to the doctors office for anything they typically take your weight and how  tall you are and they write that down in their records, well from that they can get your BMI.  The BMI is what we are wanting to see if you have had a 40 or above for the last 5 years even while trying any diets even ones that you have made up or that the doctors have given you.  "What we want is for 5 years ago your BMI number, so have your Doctor look at your records write a short sentence or two saying you were dieting and of any diets he put you on and he knows you tried.  And then have him CALCULATE that's the big thing have him calculate your BMI for the last 5 years all we really need is just 1 BMI per year and if they are all over 40 then you have that part finished.  Now if they are 30 to 35 then you'll need other medical factors like arthritis, high blood pressure, sleep aphena, type of things wrong with you to get approved.  If it is under 30 then you'll be denied by this insurance company and you'll have to go threw a year long diet weight loss documented program with your pcp and for that we will want more information before you can be approved." When she told me that I almost cried of pure joy!!! I have that part already I am almost certain that 5 years ago my BMI was 40 because it's like over 50 right now!!!  ALL I need is to find me a Doctor that will accept  my Mental evaluation that I had in August 06 (the woman said I passed it she was at Vanderbilt one of their doctors) and I did over 6 months documented with Weigh****chers in Jan 06 to almost August 06, so I also have that done already.  Now all I need to do is see about getting a doctor.  Anyone know how to take the doctor off of our profile that is listed?  SOOO HAPPY!

Angie,     I am so sorry to hear about your problems with changing of the Insurance, but did you happen to have TennCareSelect?  If so you were transitioned to new MCO for the Middle region, as they won the contract for that area, for TennCare.  We had been told that everything that had been approved was to continue on the the new MCO.  Also call AmericaChoice and ask them to send you their information/handbook, the rules should still apply for this company as well.  I hope this helps a little.
