Hello Everyone! I hope everything is well! I have a question?? I was wondering if surgeons really require that you have the diaries for when you were trying to lose weight on your own?? Or does anyone know if Dr. Ren or Dr. Fielding require them? Please I am really curious about that subject! So if anyone has info please reply!! Daniel BMI 53

daniel~ no journal for me...but i'm in california...things are probably totally different there i would definetally ask your doctor about that one...or if you have a surgeon already...ask him hope everything is well with you keep us posted ~jen

It wasn't my Dr that required it, it was my insurance, it wasn't so much a diary but more like I needed to show history of everything I had tried and failed at. I have it saved in word, if you want an example I can forward it too you, My dr. told me it was exactly what was needed, and would really help me get approved. Here I am post op so it must have done some good. Anyone else who needs an example e mail me and I'll forward it too you. Good Luck! Stacy

Do you mean a diary like, a writing journal? Or a little book thing that just documents your weight progress?! ~Amy

In the paperwork I had to fill out for my surgeon, there were these two pages that they wanted you to fill out you're weight loss history attempts on. There was a section for regular old diets(Atkins, etc.) and pcp supervised diets and you just checked which ones you did, weight lost and gained and how long you tried. I was told that they send this page to your insurance company. If your doctor wants you to fill it out that is probably why, but you should ask, as every doctor is different. my doctor needed a list, of diets, years, how you did, etc. nothing too organized...just basics
