Insurance Approval for 15yo Daughter

Having watched me go through the surgery August 2006, my daughter, who will be 16 in November and has a BMI over 50, is interested in having RNY. My insurance is currently Kaiser, but I have the option of changing insurance plans this November. Does anyone know which insurance companies (in the DC/MD/VA area) would cover WLS for 16 year olds?
