Do you need clearance from the specialist or is the primary sufficient?

I see an endocrinologist and a hematologist, only my PCP had to give clearance.

On this same subject. After a 6 month weight loss program, which insurance required, they have come back requesting several medical tests needed including a pulmonary clearance. All the other tests I have done. So I see pulmonolgists office, have a PFT and a 6 minute walk test, then the nurse practitioner says I need a sleep study for the clearance. I am really upset because I thought once those two tests were done, I would get the clearance. So I ask if I can have the sleep study done ASAP, because I have already met my deductible and out of pocket for the year, plus I was hoping to get a surgery date around end of January. They said no, soonest appt is January 17. UGGHHH!  How many more hoops do I need to jump thru here? This is so frustrating. I am ready. I have been on Step 1 of pre-op diet for over 3 weeks and have lost 15#.  Why is this sleep study so important. I do not have a known sleep apnea...they said I have all the risks of having one.
