I tried to add you as a friend to send you a questionnaire and you must be private because it wouldn't let me add you.  Add me and I will send you one because I have 3 others who also want to join so I can add all of you for May and June.  I'll send the questionnaire as soon as you add me and you will need to return it ASAP.  Thanks.
