NEW Questionnaires sent out to everyone I have signed up - Canadians contact me

Since its a New Year, I am sending out new questionnaires to everyone so I can dump my inbox and start anew.  If you did not receive one and are going to participate, please contact me.  Please fill these out and return to me ASAP so I can get new pals assigned next week.
