I can help you because I recently asked the same question. You need to email Chris at [email protected], she will then email you a questionarre to fill out. I did that and am now waiting for to be assigned a person. I am also going tohave the lap band, I am going through the supervised nutri/med 6 month and will be done in Sept. My surgeon is hoping for a Oct date. Hope that helps, Joy
