Can't remember my surgeon from 20 years ago, Middletown, Ohio

This is embarassing. I can't remember the name of the surgeon who performed my Rouxen Y back in the late 1990's/early 2000's. I know that he was located near Middletown or Dayton Ohio and was extremely prolific (his practice performed 20+ surgeries on the same day as mine). I have regained all of the weight and have had multiple other health issues and surgeons over the years. My current doctors would like to have my full history, so I'm looking for info. I'm hoping that someone here may be able to help. TIA
