Plate size
At a certain point, did you bump up your plate size? I've used a saucer (dessert plate?) since surgery. But the dietician said I can eat up to 4 oz of protein and a quarter cup of veggie. It may seem a silly question but did you up your plate to a salad plate? I've tried to do everything by the "rules" I was given but I'm afraid that the extra space on the plate could be a slippery slope.

I'm long post op, but I still tend to get smaller plates, no dinner plates.
I overload them now, unfortunately, and I can eat much more on my normal - big pouch days. On my regular or small pouch days, a few bites, and I'm done.
Overeating - eating more than my pouch allows, still can cause a severe discomfort - and or pain.
I learned to put the food away once I am done eating. Move it away from being near me if I think I'm done, but there is still food on the plate.
Once in a while I chose to eat smaller portions, or when my gut is not happy, get more slider type foods, or liquids to reset my pouch. The reset - doesn't shrink the pouch, it can make it more sensitive to the "I'm full" signal. And the full signal doesn't mean "stuffed".

I still use my salad plate and salad fork. It's more of an optic; the plate looks more full than actual need. I'm almost 5 years post op. I'm so used to it that I don't even think about it.

On rare occasion I use larger plates/bowls...99% of the time I use saucers or small bowls.
