Your intestines are lined with finger-like projections of tissue called villi. The villi grab food as it passes by and hold it against the wall of the intestine so that it can get absorbed by the bloodstream. After RNY surgery there are many fewer villi. So many of the calories that you do take in go through without being absorbed.
That results in the rapid and easy weight loss after RNY surgery. You are eating less and less is being absorbed. Until the body grows back a sufficient number of villi, calories cannot be accurately calculated.
The theory is that during year three after surgery, there will be enough new villi to absorb the calories that you take in. There is no test to calculate how many villi are there and how well they are performing. But there will be a period when you will no longer have to force yourself to eat to keep from losing weight. That is when calories will really matter again.
I personally just enjoyed my year of being too skinny and then after year three learned how quickly weight can reappear if I relax my eating habits.
