If you have GERD definitely RNY. I know several sleevers who have regretted not doing RNY due to GERD issues that get worse after the surgery. I had severe GERD which is why I did RNY and have not regretted it at all. I eat fine, you have to take vitamins with both surgeries. The only thing I can think of that's different is that some, and I emphasize SOME say you can take NSAIDs if needed on an occasional basis with a sleeve but like I said, only some say that. I've always been one to err on the side of caution so I wouldn't take NSAIDs with a sleeve either.

I'm 14 + years out, 100 per cent vitamin compliant (it's REALLY important!!) and healthy as a horse - with my own teeth .

If I had your comorbidities, I wouldn't even be considering the sleeve!!! Your surgeon is absolutely right with his recommendation.

Here are the recommendations from the ASMBS, the experts in bariatric surgery:

Generally speaking, you can meet these requirements with a double-dose of a regular adult multivitamin (something like Centrum), as well as additional calcium and iron.

After having surgery, you should have your blood work checked regularly, probably twice a year, and you should monitor the trends to see if there is anything in decline. It's somewhat common for people to have trouble with their iron levels, which can require additional supplementation. If things get VERY low, some people need to have IV iron to get their levels back up.

deficiencies, other than iron, aren't that common as long as you keep on top of your supplements. Some people don't do well on oral iron and have to go in for occasional iron infusions. I don't know the stats on that, but a lot of us do fine on oral iron.

as for teeth, we rarely hear about teeth issues on here - maybe once or twice a year. Given the thousands of posts on here every year, I would say dental issues from WLS are pretty rare. I talked to my dentist before I had RNY, and he told me he's never seen that in any of his WLS patients. He suspects it has to do with acid from reflux or silent GERD (both of which are more common in VSG patients than RNY patients), but he still encouraged me to keep on top of my calcium and vitamin D supplements. He also gave me a prescription for a super high-flouride toothpaste to use before bed, and he puts a fluoride varnish on my teeth every six months after my checkup. He said both of those would go a long way toward preventing any tooth issues (although I suspect he's doing it more to placate me since I was so freaked out about tooth issues - but I'm happy taking the precaution!!)

if you have any reflux issues, go with RNY. There's a decent chance VSG will make it worse (it won't for sure make it worse, but there's a chance). I had GERD prior to surgery, and I went with RNY because of that. No way did I want to take the risk of it getting any worse!
