B12 question
Sorry if this is a repeat topic but I did a quick search and didn't seem to see an answer to my specific question...
So I am taking Celebrate complete 36 with Iron 2x/day and also 500mg calcium citrate chews 2x /day per my nutritionist's recommendations (she provided me with a list of acceptable vitamin combo's post-RNY)
but the list was vague about B12. it just says "additional vitamins- B12 can be taken as sublingual or injections" but nothing about how often/what dosage is recommended, or whether it should be taken before/after meals, or combined with other meds/vitamins, etc.
The surgeon's office gave me the same sheet with the list of vitamins.
I was originally using Nascobal B12 nasal spray but my insurance only paid for it for one month apparently, and then it was a $56 copay so I decided to go a different route.
I do have a subligual B12 that I picked up, it is a high dosage, 1,000 mg and I have been taking it once a week. is this enough/too much?
I have left a message for the nutritionist but have not gotten a call back yet.
Just wondering what everyone else takes?

I started out taking sublingual B12 (5000mcg) once a day but my blood levels have been really high so I only take it 2-3 times per week now. You may have to test how much you need to take with regular blood tests.
HW: 248+, SW (RNY: 2/28/17): 244, GW (10/17): 125; LW: 115; 45# regain (19-20); CW: 135.6; new goal: 135; Plastics: Ext mastopexy, Ext abdominoplasty-5/18/2018; diagnosed w/ gastroparesis 11/20.
Once a week is probably not enough for the 1,000 of B12 -- but it's impossible to say. You just need to have regular labs drawn, and keep track of the results. Don't count on docs to do it. Your B12 may be "in range," according to them, but the trend is important. If it has dropped the last couple of times you've had labs done, it's time to increase your dose and/or frequency, BEFORE you are deficient. And, most docs say a level of a few hundred on B12 is fine, but many of us start getting symptoms if we are in the low end of the range. I like mine at about 1500.
Fortunately, B12 is very easy to bring up, unlike some other things. Just stay on top of your results.
I would add a third dose of calcium citrate. 1500 per day is the ASMBS recommendation, not 1,000. And have a DEXA scan, if you haven't already. You need a baseline to compare future scans to -- every two years. The calcium result on labwork tells you nothing about bone health. Only a DEXA will truly do that, though a PTH test helps. But many docs don't order those.
Finally, 36 mg of iron isn't much for most of us. You can coast on stored iron for a while, but if you don't stay on top of it, odds are very high you will end up iron deficient.
Oh. Most people need to supplement D3 also. Your labs should be at about 50 for that. Many need to take very high doses to keep theirs up.

I will start taking it daily and get myself some D3 and some additional Iron as well.
I was told that since I am a pre-menopausal female, my daily Calcium requirement was 1200...I asked if I should be taking it 3x but the NUT told me no since I would get the remaining 200 from other foods since I do eat enough dairy usually. She said just make sure I eat it with something that also has Vit C for maximum absorption (like strawberries).
I have my annual exam scheduled in a couple of weeks so I will ask my Dr. if she can order me a DEXA. I think since I am turning 40 this year, it's probably a good idea anyway. I'm sure I'll need the full gambit.

I just realized that the one I have is actually 5,000 mcg not 1,000, so maybe I should just take it 2x /week for now.

My clinic recommended either 500 mg once a day or 1000 mg every other day for sublingual B12 and that it can be taken with my multivitamin. I got the Trader Joe's one and it seems to work well.

I but the Trader Joe's B-12 and take 2000 daily. When I was taking 1000 several times a week I was at 500 which is considered normal but I was having numbness in hands and feet.
As Grim said you have to go by regular blood work. The first year I did them every 3 months and adjusted accordingly. Now I do at least twice a year once from Bariatric center and once from PCP. But honestly I pretty much ask for a slip every time I'm there.

I take a sublingual B12, 3000 mcg once a day. My B12 levels are good. BUt, that said, I think everyone is different.
