Cymbalta and Prednisone after RNY

I'm hoping someone out there might have some experience with either of these 2 drugs after surgery.

Cymbalta - been taking for a couple of years for Neuropathy, which I understand that even if my Diabetes goes into remission won't go away, but I've been finding it doesn't seem to be as effective. I asked the NP at my one month follow up but she suggested asking the pharamacist, spoke to the pharmacist and they said they weren't aware of anything on it but I should talk to my Family Doctor about increasing the dose? Not sure I want to do that as it can be addictive and at the low dose I've been taking it isn't an issue.

Prednisone - I have asthma and typically develop a secondary infection when I get a cold, have one now and have been making do with increasing my Symbicort to 4 times a day and 4 Ventolin a day, but I have Prednisone at home but the NP said I shouldn't take it as it can cause ulcers, but if I felt I absolutely had to because I couldn't breathe then she guessed I'd have no choice. But I usually start it when I get weazy to try to nip in the bud before it gets really bad, after all if I can't breathe I'm going to the ER.

Sometimes it seems I know more about the drugs and there interactions than the Doctors.

Would love to hear from anyone who has any experience with this.

Regarding the Cymbalta, I would check with the doctor who prescribed it. If it's a regular formulation, and not extended-release, it should be fine after surgery. I'd suggest meeting with the doctor more often after surgery to see if you need to tweak the dose based on how you're feeling-- maybe every month to six weeks for the first six months post-op.

As for the prednisone, the ASMBS has this to say:

Some surgeons advise limiting the use of NSAIDs after sleeve gastrectomy and adjustable gastric banding as well. Corticosteroids (such as prednisone) can also cause ulcers and poor healing but may be necessary in some situations. (Source)

Have you asked your surgeon, rather than the NP, about taking steroids?

Unfortunately, large hospital program so I only met the Surgeon briefly before my surgery, so not someone I can consult. I've learned over the years that I kind of need to be proactive myself as when you see a number of specialists they don't talk to one another and although your family doctor is technically the point person, she isn't the responsible for may of the decisions being made, frustrating but there it is. I'll ask my Respirologist the next time I see her in a couple of months as she's had a number of patients who've had surgery so will probably be able to make an informed decision.

I can only speak to the steroids as I have asthma too. I have managed to get by without them. I was in a bad way at New Years and Doc was like breathing trumps stomach. She did say make sure I was taking my PPI when taking it. I've asked at the Bariatric office too and they said the same thing, I was far enough out and if I needed to take it make sure I was taking a PPI.

That being said I filled the script but Doc said I could wait if I wanted a day or two and see if it made a turn for the better. I never did take them.

I read your post twice before I resolved my confusion about what Trump's stomach had to do with your asthma!

I'm on low dose prednisone for the rest of my life. I don't take a protective stomach medicine but I have had periods of needing high dose steroids and I do take protective medicine with that.

It's a hard drug to replace. I think you are on the right path talking to the lung guy.

Yeah I think im going to be ok this time with out but having had some scary episodes in the past I've been erring on the side of caution the last couple of years. What is ppi I take rabreprazole twice daily now as I had gerd before surgery I think the weight loss so far has helped to.

I took Cymbalta for a short period due to chronic pain. I gained weight quickly and it was a low dose. I went off and tried something else, which I am fine. That was my experience
