The only way to check to see if such a thing has occurred is to see your surgeon.
Edited for my word stupidity. :)

Your pouch is probably still working just fine. You haven't regained that much- a bit of bounce back is normal ( 10 lbs or so ) so really its just 15 lbs of regain that is relatively easy to take off with regular movement, posting your food , drinking more liquids ... ( not drinking with meals ) ..controlling the eating of slider foods ..stuff like that .
Try posting on here for a few days and also reading Kathy S 's post on losing regain. She has a lot of experience with this issue.
I'm sure your pouch is fine.
Get back to the basics ... Generally at least 65g of Protein, Less than 40g of Carbs, Less than 800 Calories, and at least 64oz of water. Weigh and measure everything, log your food in MyFitnessPal.
I worried about my pouch at one point, but when I eat dense protein (chicken, beef, pork), I feel restriction after about three ounces.
