RH strikes again
Every doctor is different and everyone's RH is different, but crackers and PB would never work for me when I get down in the 30s. I need a quick hit of sugar NOW. Glucose tabs, caramels, hard candies, juice (about 12-15g worth . . . which sucks for me since I dump) followed by a protein snack. The crackers and PB would work for my 20 minutes later follow up, but not when I'm trying to raise my sugar. I typically use mini kind bars for my follow up food and I can sometimes avoid the roller coaster highs and lows.
Peachpie, my mom has severe RH and her NP has her carry Jason's brand of portable natural almond butter/honey or peanut butter/honey. They are small packets(maybe 1oz?) that open easy, she just squirts them into her mouth. I have found them at every health food store and health sections of our grocery stores. She was carrying around glucose tablets, but these work so much better with the protein/honey combination. It's scary to watch it happen to her! I hope this helps some.
Peanut butter on a cracker seems to be the single best available treatment. Try to avoid the high sugar route that diabetics use, such as OJ and glucose tabs. It will just trigger ongoing RH, and your blood sugar will stay low. Protein will raise it, and keep it where you want it.
And of course for new folks reading this, the absolute best way to avoid RH is to avoid simple carbs. As you get a year or 2 out from surgery, they'll really start to bite you on the ass.

I've never had RH but was a diabetic on insulin for 21 years (now treated with diet and exercise). My lowest low was a 21! I had just had a baby and my hormones were jacked up. It was scary. I've known other women with BS's into the teens. I think typical glucometers stop reading around 18 and I've been with people who have had it just say 'low'.
Strictly speaking, I believe RH is different in that you can trigger another episode by treating it with straight simple sugars. But, even for my 'normal' diabetes, my endos have always been proponents of carbs + protein. Peanut butter and crackers, cheese and crackers, deli meat and crackers, a glass of milk. You can buy those pre-packaged crackers and PB/cheese and treat them like medicine (ie: they aren't to eat unless you're truly low).
I will say though that with a BS of 38 it's likely that you needed the simple sugar and then to chase it with some protein. I never found anything but juice/glucose tabs/candy to bring me up quick enough when I was extremely low. Plus, being that low always triggered what we called an 'eat the fridge' sensation and if I tried to avoid the simple sugar I would eat whole tubes of ritz crackers and peanut butter before I could stop myself!
