I agree with Kim. If my ferritin was that low, I would be getting an appointment with a hematologist.
Your B12 and D3 aren't terrible, but both are half of what I prefer. If they have been going down since your last labwork, they'll likely continue going down, and you'll end up too low, and might need to increase supplements. If they are up since your last labwork, they likely will climb a little more, and you could be ok. Trends really matter.

thank you, I appreciate the feedback because obviously my PCP is not concerned however my lack of energy is most likely due to my ferritin level. what can I take to boost it up quick?
To be honest I have not been the best with keeping up with my vitamins which is embarrassing and not acceptable on my part.. I need to get back on track!!

AN INFUSION.
You are far to low to simply bring that level back up with just a supplement. Get a hematologist appointment asap.

I am anemic to the point i have had Iron transfusion (by the way its not as scary as it sounds. Its a simple iv hooked up to saline then they push the iron then let the saline sit 5 more minutes then boom done.)
But I would talk it over with your doctor and see if the hematologist needs to get involved. my Ferritin (iron stores) have been way low like that but she always looks at the overall not just ferritin she watches the trend in the iron levels even b12 levels and makes that determination if i need an infusion or a round of them. Your tsh (hypothyroidism watch) is creeping up but if its like mine you can go to 4.00 i think before they even worry about it.
As far as vitamins its a matter of taste preference. I used Baratric Fusion for a while but all i could handle is orange cream. I have since switched to Flinstone Completes ,calcium citrate, and i get my vitD weekly from my doctor and B12 shots monthly.
It wouldn't hurt to talk to your surgeon or Dietitian the surgeon uses. Sometimes they have samples of different vitamins for you to try. Its worth a shot.
You need a good bariatric vitamin. This is very important. Even with doubling how many Centrum you take you're not getting the full amount the ASMBS recommends. Those numbers are very important. I agree with the others on the other recommendations. I take Bari-Life vitamins, but there are other brands that are good, too. Bari-Life is probably less expensive than doubling the Centrum.

Ok, unfortunately the iron infusion is not going to be possible thanks to my lovely marketplace health plan, they do not cover until my deductible is met and iron infusions cost around $2,700 per session out of pocket. What can I do now, what iron supplement can I take to boost my numbers quickly? thanks!
