Any medical advice should be provided by your doctor, so I would get the recommendation from their office.  If I were personally taking it, I would start with 45mg of the OTC and see first.  There is a risk trade-off on side effects as you climb the dosing curve.  That's why products get approved at specific doses.  As I recall the dose ranging studies showed the difference between 45 and 60 of lanso wasn't very significant.  I always though the Dexilant dose would have been labeled at 45mg, sort of surprised it went to 60.

I would be interested to see if Prevacid works for you. It did the same as omeprazole for me . . . Nothing

You guys rock! I have tried samples of Dexilant (insurance won't cover it) Omezerpole, Prilisec, Zantac, Pepcid, Tagamet, and Protonix with little to no relief. I've even ended up being monitored in the ER for my heart because it was so bad one evening I really thought I was having a heart attack (49 year old former smoker).

I'm going to start pursuing the suggestions Stacyrg gave and finally get off the fence and do something. I really appreciate all the suggestions! Makes me want to cry because I'm becoming desperate :(

Pursuing revision was the best thing I ever did for myself. I haven't had the easiest of transitions but it beats choking on acid every single day. Feel free to keep I touch if I can offer any more support or answer any questions!
